Job Summary:

The Physical Therapist will be responsible for providing high-quality physical therapy services to patients in their homes. This includes assessing patients' conditions, developing personalized treatment plans, and delivering therapy to improve mobility, reduce pain, and restore function.

Key Responsibilities:

  • Conduct comprehensive assessments of patients' needs, including OASIS assessments.

  • Collaborate with interdisciplinary teams to ensure comprehensive care.

Requirements:

  • Graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association.
  • Licensed or registered by the State.
  • At least one year of institutional experience.
  • Home care experience and supervisory experience preferred.
