As a Social Worker in our Hospice and Home Care team, you will play a vital role in providing therapeutic intervention and social work services to patients and their families. Your goal will be to enhance comprehensive, integrated, and uninterrupted care in the hospital and ensure continuity of care in the community.

You will assess patients' psychosocial situations in relation to their medical needs and plan appropriate interventions and treatment goals to improve patient/family functioning. This will involve interviewing patients, family members, and/or significant others to assess needs for social work and/or interventions related to social determinants of health.

You will also function as a resource and advocate on behalf of the patient and family for discussions involving Advanced Care Planning. Additionally, you will provide preventive, protective, or supportive services to patients who are in or threatened by situations detrimental to their well-being.

Collaborating with the healthcare team regarding in-hospital needs and post-hospital care will be essential. You will maintain current with available psycho-social services programs/services and provide relevant information to patients, families, staff, and community organizations. Developing and maintaining collegial relationships with community organizations will also be a key aspect of this role.

You will coordinate and maintain a viable psychosocial discharge planning program for the assigned service area to identify patient needs and plan for appropriate after-care services. Ensuring continuity of care through comprehensive and appropriate discharge planning with patient, family, and/or significant others will be a top priority.

  • Long Term/Custodial Care
  • Acute rehab, Sub-acute rehab/Skilled Nursing Facility (SNF)
  • LTAC, Dialysis, Hospice
  • Homelessness solutions and resources

You will document appropriate data from psycho-social assessments, individual contacts, family interventions, and discharge planning according to department policy and procedures and State, Federal, and Joint Commission stipulated requirements.

Developing a social service network with city, state, or other governmental services that serve the needs of the assigned patient population will be crucial. This will involve assisting patients in obtaining services such as food stamps and other financial supports and assistance in obtaining transitional housing.

Evaluating psycho-social services program objectives and results and participating in program planning and improvement in accordance with the mission of the hospital and department will be essential. Maintaining confidentiality in accordance with hospital and departmental policy and professional standards will also be a key responsibility.

This role requires a Graduate of an accredited school of Social Work (MSW) and previous hospital medical social work experience is strongly preferred.
