Physical Therapist Home Health Care Specialist

BAYADA Home Health Care is seeking an experienced Physical Therapist to join our team in providing high-quality home health care services to our clients in the Lower Bucks County and Northeast Philadelphia area.
About the RoleAs a Physical Therapist with BAYADA, you will be part of a multidisciplinary team that provides rehabilitative care and skilled nursing to clients in the comfort of their own homes. You will work closely with our clients, their families, and other healthcare professionals to develop and implement personalized care plans that meet their unique needs and goals.
Responsibilities- Conduct thorough assessments of clients' physical abilities and develop individualized care plans to address their needs
- Provide skilled physical therapy services to clients in their homes, including exercises, stretches, and other interventions to improve mobility and function
- Collaborate with other healthcare professionals, including nurses, occupational therapists, and social workers, to ensure comprehensive care
- Communicate effectively with clients, families, and other healthcare professionals to ensure seamless care coordination
We are seeking a licensed Physical Therapist with at least one year of prior clinical experience. You must be able to work independently and as part of a team, and possess excellent communication and interpersonal skills.
