As a Registered Nurse with Geisinger Health System, you will play a vital role in providing high-quality care to our patients. Your primary responsibility will be to perform client assessments and triage, coordinating referrals to in-house clinical and community services as needed. You will also conduct family education, provide all aspects of nursing care, and assist physicians and physician extenders in the diagnosis and treatment of patients.
Key Responsibilities:
- Enroll clients into the Healthy Beginnings Program and oversee their overall care.
- Provide primary clinical support to providers, patients, and their families to assist in diagnosis and treatment.
- Follow up on diagnostic testing to ensure completion, accuracy, and filing in medical records.
- Develop care coordination records and initiate appropriate referrals.
- Maintain comprehensive problem lists and update at each visit.
- Complete trimester education and encourage active participation from clients.
- Provide education to improve maternal and neonatal outcomes.
- Serve as a resource for the initiation of additional sites.
- Provide reporting to regulatory agencies as required.
- Assist with coordinating external disease management and case management organizations.
Requirements:
- Graduate from a Specialty Training Program.
- Minimum of 2 years of nursing experience.
- Basic Life Support Certification and Licensed Registered Nurse (Pennsylvania) certification.
