Job SummaryWe are seeking a qualified Healthcare Specialist to join our Weekend Admissions team in Street, Maryland. As a member of our team, you will play a critical role in delivering exceptional patient care and ensuring the highest level of satisfaction for our clients.
Responsibilities:- Conduct thorough assessments of patients' needs and develop personalized care plans to meet those needs.
- Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by physicians.
- Maintain accurate and detailed records of patient progress and communicate effectively with patients, families, and healthcare teams.
- Collaborate with interdisciplinary teams to coordinate patient care and ensure seamless transitions between healthcare settings.
- Participate in ongoing education and professional development to stay current on best practices and advancements in home health care.
To be considered for this role, applicants must possess:
- A current license as a Registered Nurse (RN) in Maryland.
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
- Prior home care experience is strongly preferred but not required.
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
- Ability to work independently and manage time effectively.
- Strong interpersonal skills and ability to build rapport with patients, families, and healthcare teams.
