Practical Nurse

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Right at Home Eastern Worcester County. As a key member of our care team, you will provide high-quality nursing care to seniors and adults with disabilities in the comfort of their own homes.
Responsibilities- Provide personalized nursing care to clients, including administering medications, wound care, and vital sign monitoring.
- Conduct thorough assessments of clients' physical and emotional needs, and develop individualized care plans to meet those needs.
- Collaborate with clients' physicians and other healthcare professionals to ensure seamless care coordination.
- Supervise and train caregivers to ensure they are providing high-quality care.
- Document client progress and communicate with clients, families, and healthcare teams as needed.
- Graduate of an accredited practical or vocational nursing program and licensed to practice in the state.
- 3 years of nursing experience, with a preference for home health/hospice experience.
- Knowledge and experience working in homecare, with a strong understanding of gerontology and adult health.
- CNA/PCA experience a plus.
- Strong communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare teams.
- Ability to work independently and as part of a team, with a strong commitment to providing exceptional client care.
