Job Title: Psychosocial Rehabilitation Provider

At Access, we are seeking a dedicated and compassionate Psychosocial Rehabilitation Provider to join our team. As a key member of our organization, you will play a vital role in supporting children and youth with their developmentally appropriate functioning, enabling them to integrate into their families and communities.

Job Summary:

The Psychosocial Rehabilitation Provider will provide community-based services to children and families who may have difficulty engaging in formal office settings. Your expertise will focus on restoring, rehabilitating, and supporting the child/youth's developmentally appropriate functioning, promoting their active and productive participation in their family and community.

Key Responsibilities:
  • Provide community-based services to children and families, adapting to their unique needs and circumstances.
  • Develop and implement individualized rehabilitation plans, incorporating social and interpersonal skills, daily living skills, and community integration.
  • Collaborate with multidisciplinary teams to ensure comprehensive support for children and families.
  • Conduct assessments, evaluations, and progress monitoring to inform rehabilitation planning and goal setting.
  • Maintain accurate and timely documentation of services provided, progress, and outcomes.
Requirements:
  • Minimum 18 years old with a high school diploma or equivalent, and a minimum of 2 years of experience in children's mental health, addiction, foster care, or related human services.
  • Valid driver's license and dependable transportation.
  • Ability to complete Mandated Reporting self-directed online training.
  • Willingness to work late afternoons, evenings, and some weekends.
