Position Overview

The Chronic Care Registered Nurse is responsible for delivering and coordinating high-quality clinical care for a designated group of patients. This role encompasses various responsibilities, including:

  • Patient Assessment and Care Coordination: Conduct thorough evaluations, develop care plans, implement interventions, and assess outcomes for patients undergoing hemodialysis.
  • Team Collaboration: Work closely with the End Stage Kidney Disease (ESKD) healthcare team to participate in decision-making processes, educational initiatives, and quality improvement projects aimed at enhancing patient care.
  • Delegation and Supervision: Oversee and delegate tasks to nursing staff, including RNs, LVNs/LPNs, and Patient Care Technicians, ensuring adherence to established protocols and standards.
  • Data Management: Perform systematic collection and analysis of patient data before, during, and after hemodialysis treatments, documenting findings in the medical record and adjusting treatment plans as necessary.
  • Patient Education: Assess and document the educational needs of patients and their families, providing ongoing instruction regarding treatment options and care processes.
  • Medication Administration: Administer prescribed medications in accordance with established guidelines, ensuring proper documentation of medical justifications and patient responses.
  • Emergency Response: Be prepared to initiate or assist with emergency measures as required.
  • Training and Resource Provision: Serve as a resource for the healthcare team, contributing to staff training and the orientation of new personnel.
  • Laboratory Procedures: Ensure compliance with laboratory collection, processing, and shipping protocols, rescheduling any missed or inadequate collections.
  • Interdisciplinary Collaboration: Identify expected patient outcomes and collaborate with the Interdisciplinary Team to update nursing assessments and care plans.
  • Patient Awareness: Facilitate patient understanding of transplant and treatment modality options.
  • Additional Responsibilities: Carry out other duties as assigned by the Supervisor.
Supervisory Responsibilities

This role may include oversight of Patient Care Technicians, LPNs, LVNs, and RNs, designated as Team Leader or Nurse in Charge, contingent upon:

  • Successful completion of all required training and education for new employees.
  • A minimum of 9 months of experience as a Registered Nurse.
  • A minimum of 3 months of experience in chronic or acute hemodialysis as a Registered Nurse.
