Job Title: Family Nurse Practitioner

Harbor Health Services, Inc. is seeking a highly skilled and compassionate Family Nurse Practitioner to join our Primary Care Department at the Daniel Driscoll Neponset Health Center in Dorchester, MA.

About the Role:

The successful candidate will provide high-quality, patient-centered care to a diverse population, including routine health maintenance, management of chronic conditions, and management of acute illness. This is a full-time position, with the possibility of evening shifts and Saturday rotations based on clinic needs.

Responsibilities:
  • Provide primary care to a panel of patients, including outpatient visits, communication with patients, inpatient care, and referrals for specialty care.
  • Organize and manage the care of primary care patients, including health risk assessments, disease prevention, and evidence-based medicine.
  • Perform routine health maintenance, including well visits, cancer screening, and health promotion education.
  • Manage common chronic illnesses, including acute complications and routine follow-up.
  • Participate in an on-call system and provide 24-hour access to a provider for our patients.
  • Participate in evening/Saturday/holiday clinic schedule to provide face-to-face care, follow-up visits, acute care, and other clinical services.
  • Participate in provider staff meetings, departmental meetings, trainings, care team meetings, and quality improvement/practice transformation sessions.
  • Update Electronic Medical Record (EMR) appropriately and accurately document care in the medical record.
Requirements:
  • Completed accredited Nurse Practitioner program in Family Practice or Adult Medicine.
  • Current MA NP License, Family Practitioner, Board Certified, Current DEA, MCSR, and Basic Life Support for Healthcare Providers (BLS) required.
  • Computer Literate (MS Office, Windows), experienced with EMR (EPIC/OCHIN highly desired).
  • 3-5 years of NP experience required and bilingual preferred.
  • Experience with a community health center population and practice setting preferred.
