Key Responsibilities:

  • Provide skilled nursing care to patients in their homes, focusing on comfort, dignity, and quality of life.
  • Conduct thorough assessments, develop care plans, and implement interventions to meet the unique needs of each patient.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
  • Document patient progress, identify areas for improvement, and participate in quality improvement initiatives.
  • Maintain accurate and up-to-date patient records, adhering to regulatory requirements and company policies.

Requirements:

  • Current LPN license in good standing, with a valid state license.
  • At least one year of experience in an acute care or equivalent setting, with a strong foundation in nursing principles and practices.
  • CPR certification with the American Heart Association or American Red Cross.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Reliable transportation to perform job duties, with a valid driver's license and a safe driving record.
