**Responsibilities:**
- Provide skilled nursing care to patients in their homes, including wound care, medication management, and education.
- Conduct thorough assessments of patients' physical and emotional needs, developing personalized care plans to meet their unique requirements.
- Collaborate with healthcare teams, including physicians, therapists, and social workers, to ensure comprehensive care and coordination.
- Document patient progress, including medical history, treatment plans, and outcomes, in accordance with regulatory requirements.
- Maintain accurate and up-to-date records, ensuring confidentiality and compliance with HIPAA standards.
- Participate in ongoing education and training to stay current with best practices, industry developments, and regulatory changes.
**Requirements:**
- Current Florida RN License
- Minimum of 1 year of recent home health nursing experience (private duty does not qualify)
- OASIS/Medicare Certified Visits Experience
- Electronic Charting experience
- American Heart Association BLS
- Valid U.S. Driver's License
