Care Coordinator for Aging Adults

At Lutheran SeniorLife, we offer a comprehensive continuum of care for older adults, providing a range of services from residential living to skilled nursing. As a Care Coordinator for Aging Adults, you will play a vital role in helping families navigate our care options and ensuring their loved ones receive the support they need to live healthy, happy, and safe lives.
We are seeking a highly skilled and compassionate Social Worker to join our interdisciplinary team. As a Care Coordinator, you will be responsible for identifying and developing care plans that address the psycho-social needs of our residents/patients. This will involve working closely with residents/patients, their families, and our healthcare team to ensure that each individual receives personalized care and support.
Key responsibilities will include:
- Developing and implementing care plans that meet the unique needs of each resident/patient
- Acting as an advocate for residents/patients and their families, ensuring that their voices are heard and their concerns are addressed
- Providing education and support to residents/patients and their families, helping them to navigate our care options and make informed decisions about their care
- Collaborating with our healthcare team to ensure that residents/patients receive high-quality, person-centered care
We offer a competitive salary, with an estimated annual salary of $60,000 to $80,000 depending on experience, plus a range of benefits including medical, dental, vision, and life insurance, as well as opportunities for career growth and advancement.
Requirements:
- CURRENT LICENSE under the Social Workers, Marriage and Family therapists, and Professional Counselors Act, or a Bachelors, Masters, or doctoral degree from an accredited school or program in social work or social welfare
- 1 year experience in health or related field
- Strong written and verbal communications skills
- Working knowledge of computers, Microsoft Word, and Excel required
