Responsibilities:
- Perform evaluations and develop treatment plans that cater to each patient's unique needs and goals
- Regularly re-assess effectiveness of treatment plans, attend staffing meetings, and communicate with patients, families, and healthcare teams
- Perform discharge evaluations, make recommendations for home and community safety, and provide referral and education resources
- Communicate patient needs and progress to the treatment team, physician, and family members
- Collaborate with other disciplines to ensure coordination of care and enhance patient outcomes
Requirements:
- Minimum of bachelor's degree in Physical Therapy from an accredited program
- Current, unrestricted license as a Physical Therapist in the state where practicing
- Current Basic Life Support or Cardiopulmonary Resuscitation certification, as required by state regulations
- A minimum of one year's work experience as a physical therapist, with a preference for those with neurological experience
- Demonstrated knowledge of rehabilitation techniques related to complex neurological injuries
- Effective communication and interpersonal skills, with the ability to work with diverse populations
- Ability to lift 50 pounds and perform physical demands of the job, with occasional lifting of up to 100 pounds
