**Job Summary**
Centra Healthcare Solutions is seeking an experienced Physical Therapist (PT) to work in the specialty area of Skilled Nursing Facilities (SNF) and Long-Term Care (LTC).
This is a contract, 5x8 Days position that requires the candidate to be licensed or in the immediate process of obtaining Physical Therapist licensure in the state of North Carolina.
**Responsibilities**
- Assume responsibility and accountability for incorporating the vision, values, mission, and critical goals of the organization into their job performance.
- Provide high-quality patient care and services in a SNF/LTC setting.
- Collaborate with the healthcare team to develop and implement individualized patient care plans.
- Conduct thorough assessments and evaluations to identify patient needs and develop effective treatment plans.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
**Requirements**
- 1 year of recent experience as a Physical Therapist specializing in SNF/LTC.
- Current Physical Therapist licensure within the state of practice.
