POSITION SUMMARY
As a Registered Nurse at LifeBridge Health, you will play a vital role in delivering compassionate and comprehensive nursing care to patients in their homes during the final stages of life. Your responsibilities will include:

Key Responsibilities:
1. Assessing the needs of patients and their families to develop, implement, and evaluate individualized care plans.
2. Providing professional nursing care in alignment with the organization's Mission Statement and core values.
3. Performing primary care case management for assigned patients, ensuring continuity of care.

QUALIFICATIONS:
- Required: Associate's Degree in Nursing from an accredited institution.
- Preferred: Bachelor's Degree in Nursing.
- Minimum of two years of recent medical/surgical nursing experience; home health experience is a plus.
- Certification as a Certified Hospice and Palliative Nurse (CHPN) is required within four years of hire.
- Current CPR certification as an AHA Healthcare Provider.
- Active Registered Nurse (RN) licensure in Maryland and Pennsylvania.

ADDITIONAL REQUIREMENTS:
Excellent interpersonal communication skills, both written and verbal, are essential for educating and comforting patients and their families. You will maintain effective communication with various healthcare personnel and ensure accurate documentation of patient care.
