Catholic Health is seeking a skilled Registered Nurse to join our Home Care team on a weekend shift basis. As a Home Care RN, you will be responsible for providing high-quality patient care in the comfort of their own homes.
Key Responsibilities- Case manage, assess, plan, develop, and implement comprehensive home health care plans for patients.
- Measure vital signs, administer IVs, change dressings, and clean wounds as needed.
- Provide skilled nursing care, preventative rehabilitative procedures, and prescribed treatments to a variety of patient populations within complex home situations.
- Work closely with a supportive patient care team, including physicians, therapy, and social services, to ensure seamless care delivery.
- Provide patient and family education on disease management and health promotion, as directed by the physician's plan of treatment.
- New York State RN license.
- Minimum of one year of recent hands-on experience as a Registered Nurse.
- Basic Life Support Certification.
- Valid New York State Driver's License and reliable transportation.
