We are seeking a skilled Licensed Practical Nurse (LPN) to join our team in providing high-quality care to patients in their homes. As a Home Care LPN, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
Key Responsibilities- Provide direct nursing care to patients in their homes, as prescribed by physicians.
- Record clinical and progress notes, and coordinate other patient care activities to ensure the highest quality of patient care.
- Work collaboratively with the healthcare team to develop and implement individualized care plans.
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
- Current LPN licensure in the state of practice.
- Minimum of one year of acute care experience, with previous home care experience preferred.
- Current CPR certification.
- Ability to work flexible hours, including evenings and weekends, as needed.
