Job Summary:
We are seeking a skilled and compassionate Home Health Nurse to join our team at Mercy Hospital Oklahoma City. As a Home Health Nurse, you will provide high-quality patient care in the comfort of patients' homes, working closely with our interdisciplinary team to ensure seamless care transitions.
Responsibilities:
- Provide day-to-day patient care in the home setting, including case management, total care of multiple patients, admissions, recertifications, discharges, patient teaching, and dressing changes.
- Administer IV therapy, access site care, and general skilled nursing according to policy and procedure.
- Assist with other needs apparent in the home to ensure safety and appropriate care, such as lifeline, meals on wheels, and instruction on public programs availability.
- Document patient care and progress using the Epic computer system in a timely and accurate manner.
Requirements:
- Nursing Degree from an accredited school.
- Oklahoma RN licensure and valid driver's license.
- CPR certification and Basic Life Support certification through the American Heart Association or successful completion of course within 30 days of hire.
- LPN-IV Certification must be previously completed or successful completion of course within 3 months of hire.
- Prefer one year experience in a related field.
