Job Overview:

Compensation: $100,000.00 annually

The Advanced Palliative Care Nurse Specialist is responsible for delivering and documenting essential medical services in alignment with established physician and nurse practitioner protocols.

This role involves formulating comprehensive treatment strategies aimed at optimizing patient health outcomes, which includes the ordering and interpretation of diagnostic evaluations, selecting suitable pharmacological treatments, identifying effective non-pharmacological interventions, and developing educational plans tailored to patient needs.

Key Responsibilities:

1. Engage in advanced care planning to ensure seamless continuity of care.
2. Collaborate with a network of healthcare professionals, including consulting physicians, primary care providers, and specialists.
3. Foster relationships with hospital-based palliative care teams, partner organizations, and community healthcare providers to cultivate a collaborative care culture.
4. In partnership with primary care providers, refer patients to medical specialists and other healthcare services as clinically indicated.
5. Assist in coordinating care for referrals and manage outreach initiatives in collaboration with the marketing department.
6. Provide disease management education and support to patients and their families.
7. Offer clinical guidance to facility staff regarding patient care assessments and interventions.
8. Participate in on-call services as required.
9. Collaborate with interdisciplinary teams in various care settings.

10. Maintain accurate patient records and documentation to support effective medical practice and ensure appropriate reimbursement for services rendered.


11. Serve as a clinical resource for complex case management to facilitate safe transitions to other care environments.
12. Adhere to all relevant laws and regulations concerning delegation protocols, prescriptive authority, and nurse practitioner scope of practice.
13. Review policies and services with patients and their families, ensuring informed consent for medical care.
14. Work closely with the intake department to verify coverage and initiate reimbursement processes.
15. Contribute to the development and evaluation of clinical practice guidelines to enhance quality care and support policy formulation.
16. Uphold integrity, compassion, and a customer-focused approach to ensure high-quality care delivery.
17. Comply with all employment policies and maintain confidentiality in accordance with HIPAA regulations.
18. Participate in mandatory educational programs and meetings.
19. Adhere to the dress code policy, maintaining a professional appearance while on duty.
20. Perform additional duties as assigned.

Qualifications:

Required Skills and Abilities:
1. Comprehensive understanding of palliative care principles and home-based medical service delivery.
2. Knowledge of the end-of-life process and hospice organization policies.
3. Proficient in clinical and administrative tasks necessary for the role.
4. Ability to navigate state regulations regarding practice agreements with physicians.
5. Capacity to support patients and families during challenging times.
6. Skill in assessing patient eligibility for hospice participation.
7. Ability to cultivate constructive relationships with colleagues and external organizations.
8. Excellent communication skills for effective information exchange between referral sources and clinical teams.
9. Demonstrate respect and accountability towards the public, patients, families, and coworkers.
10. Maintain a professional demeanor and positive representation of the organization.

Education and Experience:

Required Education:
Licensed nurse practitioner with board certification, authorized to practice in New Jersey. Certification in Hospice and Palliative Care is mandatory. Compliance with the Department of Public Safety and Drug Enforcement Agency regulations is required.

Preferred Education:

Required Experience:
Experience in family practice, oncology, or hospital settings is essential.

Preferred Experience:
Prior experience as a nurse practitioner is advantageous.
