Job Description

The Salvation Army Central Territory is seeking a skilled Case Specialist to join our team. As a Case Specialist, you will play a vital role in providing transactional services to low-income applicants, assisting them in meeting their basic needs and promoting family sufficiency.

Key Responsibilities:
  • Meet with applicants, assess their needs, and determine eligibility for resources.
  • Develop plans to address presenting problems and provide information on available resources.
  • Facilitate case management using the Pathway of Hope guidelines and assessment tools.
  • Maintain working relationships with community agencies and promote Pathway of Hope.
  • Coordinate services, communicate about cases, and ensure valid releases of information are on file.
  • Help participants achieve identified goals through agency resources and referrals.
  • Enhance personal knowledge and skills through agency and community educational forums.
  • Manage case records, service transactions, and fund balances.
  • Provide information for reports to administration and perform other duties as assigned.
Qualifications:
  • Bachelor's degree in social work, psychology, sociology, or a related field.
  • 1 or more years of applicant interview or case management experience.
  • Experience working with low-income populations is preferred.
