Job Title: Nutrition Specialist

We are seeking a highly skilled Nutrition Specialist to join our team at Concordia Lutheran Ministries and Affiliates. As a Nutrition Specialist, you will play a vital role in improving the diet, nutrition, and health of our home health patients.

Responsibilities:
  • Provide nutritional counseling to patients with medical problems affected by nutrition status
  • Develop and implement individualized diet therapy plans for patients
  • Conduct nutritional screenings and assessments for assigned patients
  • Act as a nutrition resource person on the healthcare treatment team
  • Plan and implement in-service educational programs for clinicians on nutritional topics
Requirements:
  • Graduation from an accredited college or university with an advanced degree in nutrition or its equivalent
  • Ability to communicate effectively with patients, families, and healthcare staff
  • Knowledge of nutritional care and counseling principles and practices
  • Ability to maintain patient and staff confidentiality
