Job Summary
This position is responsible for ensuring the optimal use of medications to achieve specific outcomes that improve a patient's quality of life.
The pharmacist will perform various functions, including dispensing and distributing drugs, advising physicians and patients on proper usage, and collaborating with other healthcare professionals to plan, monitor, and evaluate patient effectiveness.
The pharmacist will also review medication orders for appropriateness and accuracy, interpret and clarify physician medication and IV orders, and select medications for outpatient dispensing.
Additionally, the pharmacist will monitor patients on specified target drugs, initiate pharmacokinetic dosing protocols, evaluate drug therapy for appropriateness, and make drug therapy recommendations to clinicians.
The pharmacist will also prescribe drug therapy per collaborative drug therapy management agreements and perform patient counseling.
Requirements
- Pharmacist - Oregon currently licensed or able to obtain license
- Demonstrated basic pharmacy clinical skills and customer service skills
