Job Summary

Family Residences and Essential Enterprises, Inc. is seeking a highly motivated and compassionate Peer Specialist-OMH HCBS to join our team. As a Peer Specialist, you will play a vital role in supporting individuals with mental health conditions to achieve their personal and independent living goals.

Key Responsibilities
  • Provide supportive services to individuals in OMH Supportive Housing, Mobile Residential Support Team (MRST), and OMH Crisis Programs.
  • Develop and implement personalized support plans to enhance self-confidence, self-responsibility, and personal growth.
  • Assist individuals in transitioning from one setting to another (bridging) and provide crisis management and risk assessment services.
  • Collaborate with treatment teams, clinicians, and collateral providers to ensure comprehensive care.
  • Participate in incident reporting, investigations, and quality assurance activities.
  • Ensure compliance with New York State regulations and Agency policy.
Requirements
  • High School Diploma required.
  • NYS Peer Specialist Certification.
  • Valid NYS Driver's License and ability to drive for the organization.
  • Ability to communicate effectively orally and in writing.
  • Ability to work independently with minimal supervision.
Work Environment

This role is performed in-person, with travel between locations within the FREE Network required. The Peer Specialist-OMH HCBS will work in various settings, including community-based locations, housing units, and other places agreed upon by the individual and employee.

Physical Requirements

The employee is required to sit, walk, and stand for prolonged periods (at least 8 hours). They must also be able to effectively communicate verbally and in writing, operate standard office equipment, and reach with hands and arms to lift/move light items.
