Job Summary

The Home Health LPN/LVN will provide nursing care to clients in their place of residence, under the supervision of a registered nurse. The ideal candidate will have experience in a clinical care setting and excellent interpersonal skills.

Key Responsibilities
  • Assist in identifying clients' physical, psychosocial, and environmental needs.
  • Participate in planning and implementing care in conjunction with the RN, in accordance with the Plan of Care (POC).
  • Provide effective communication to clients, their families, team members, and other healthcare professionals.
  • Monitor assigned cases to ensure compliance with third-party payer requirements.
  • Complete documentation in a timely manner to ensure compliance with company policy.
Requirements
  • 1-year experience as an LVN/LPN in a clinical care setting.
  • Preferred home health experience.
  • Nursing skills as defined by generally accepted standards of practice.
  • Excellent interpersonal skills and ability to communicate effectively.
  • Demonstrates proven decision-making skills.
  • Must read, write, and comprehend English.
Physical Requirements
  • Prolonged standing/walking is required.
  • The ability to lift/transfer up to 50 lbs.
  • Ability to meet deadlines and patient needs, make quick decisions, and allocate resources under stressful conditions.
Environmental/Working Conditions
  • Works in patients' homes in various conditions.
  • Possible exposure to blood-borne pathogens, bodily fluids, and infectious diseases.
  • Some exposure to unpleasant weather; PRN emergency call.
