Job Title: Physical Therapy Assistant

Pinnacle Home Care Inc. is seeking a dedicated and compassionate Physical Therapy Assistant (PTA) to join our team of skilled home health care providers. As a PTA, you will work under the supervision of a licensed Physical Therapist to provide physical therapy services to patients in their own homes.

Key Responsibilities:
  • Assist in the development and implementation of the interdisciplinary client care plan as it pertains to Physical Therapy
  • Treat clients to relieve pain, develop or restore functional and maintain maximum performance, using physical means, such as exercise, massage, heat, light and electricity
  • Teach, supervise, and counsel the family and client in the physical therapy program
  • Instruct client in care and use of adaptive devices such as: wheelchairs, braces, crutches, canes and prosthetic and orthotic devices
  • Arrange for out-patient service for procedures when appropriate
  • Record on a timely basis all treatments, and client's response to therapy intervention Also record quality assurance data
  • Communicate with PT and other team members (including Home Health Aides), when appropriate. Instruct them in certain phases of physical therapy that they may use while working with the client
  • Under the direction of a Physical Therapist, write clinical/progress notes on the date of the visit and incorporate same in the client's record according to company policy
  • Participate in case conferences, physical therapy staff meetings, agency activities and in-service programs as necessary.
  • Participate in effective coordination of schedule to ensure all reassessments are performed in a timely manner
  • Obtain a medical history, particularly as it relates to the present condition
  • Identify possible ineffective drug therapy or adverse reactions, significant side effects, drug allergies, and contra-indicated medication and report same to the PT
  • Observe, and record activities and findings in the clinical record and report to the PT and the physician any adverse reaction to treatment
  • Participate with staff, clients, and physicians in discharge activities and assist the Physical Therapist in completing the Physical Therapy Discharge Summary
Requirements:
  • Must be licensed by the State as a Physical Therapist Assistant
  • Must be a graduate of an approved school giving a course of not less than two years for Physical Therapist Assistants
  • Must have one year of experience in an acute/sub-acute setting
  • The ability to drive and the ability to access clients' homes, which may not be routinely wheelchair accessible are required
  • Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the client's condition and to perform client care
  • May be required to bend, stoop, reach and move client weight up to 250 pounds, lift and/or carry up to 30 pounds
