Samaritas is seeking a skilled Preservation Case Manager to join our Family Preservation and Reunification Team. As a key member of our team, you will work with at-risk families to provide intensive, short-term or long-term in-home crisis intervention and family education.
Key Responsibilities- Provide in-home crisis-oriented treatment, education, and support to family units where one or more family members may be placed outside the home in foster, group, or institutional care or, be at-risk of being removed from the home.
- Develop and implement comprehensive treatment plans, assess progress, and make recommendations for case goals, alternative service plans, and action steps.
- Collaborate with referral sources, agencies, institutions, and individuals who have important roles in the family's life situation, providing advocacy and professional consultation as needed.
- Complete client follow-up at intervals as outlined, and maintain all reports, statistics, and assignments as required.
- Bachelor's degree in a human service field or education, or two years' experience working with children and/or families and an associate degree or credit equivalent in human services or education.
- Valid driver's license with a good driving record, reliable ongoing personal transportation suitable for safely transporting clients, and current automobile registration and proof of automobile insurance.
