Key Responsibilities- Provide primary care services to geriatric patients, including routine check-ups, health screenings, and disease management.
- Collaborate with the healthcare team to develop and implement individualized care plans for patients.
- Conduct thorough assessments of patients' medical histories, medications, and health status to identify potential health risks and develop strategies for prevention and management.
- Order and interpret diagnostic tests, such as lab work and imaging studies, to inform treatment decisions.
- Prescribe medications and treatments as needed to manage patients' health conditions.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination and patient satisfaction.
- Current Maryland Nurse Practitioner License
- Current DEA License
- Current CDS License
- Experience working in geriatric care settings
- Strong clinical and interpersonal skills
- Ability to work independently and as part of a team
- Competitive hourly rate of $65.00 to $80.00
- Flexible scheduling with three days per week, Monday through Friday
