About the Role
We are looking for a highly skilled and compassionate individual to work as a Clinical Health Coordinator. This is a remote position that requires travel up to 25% of the time to Humana Healthy Horizons office in Glen Allen, VA for collaboration and face-to-face meetings. You will be responsible for assessing and evaluating members' needs, guiding them toward appropriate resources for their care and well-being. Your exceptional communication skills and ability to build rapport quickly will enable you to effectively manage complex situations and make decisions independently. In this role, you will employ various strategies to manage members' physical, environmental, and psycho-social health issues.

Required Qualifications
To succeed in this position, you must have a Virginia licensed RN with a minimum of 2 years of post-degree clinical experience in a behavioral health setting. Additionally, you should possess case management experience working with complex SMI or SED populations. A valid driver's license, car insurance, and reliable transportation are also essential requirements.
