Home Health Risk Assessment Nurse Practitioner or Physician Assistant

We are seeking a highly skilled and compassionate Nurse Practitioner or Physician Assistant to join our team at Matrix Medical Network. This PRN/Part Time position offers the opportunity to work in a variety of settings, including home visits, skilled facilities, and telehealth appointments.
About the Role:
- Conduct comprehensive health assessments of adult and geriatric patients in their homes or other community environments.
- Work collaboratively with healthcare teams to develop and implement individualized care plans that address the unique needs of each patient.
- Provide education and support to patients and their families on disease management, medication adherence, and healthy lifestyle choices.
- Collaborate with primary care physicians and other healthcare providers to ensure seamless care coordination.
Responsibilities:
- Perform thorough medical histories, physical examinations, and diagnostic testing as needed.
- Diagnose and treat a range of conditions, including chronic diseases, acute illnesses, and mental health concerns.
- Develop and implement treatment plans that take into account the patient's medical history, social environment, and cultural background.
- Maintain accurate and up-to-date records of patient interactions, including assessment findings, treatment plans, and progress notes.
Requirements:
- Master's degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) degree required.
- Current licensure as a Registered Nurse (RN) and Nurse Practitioner (NP) or Physician Assistant (PA) in the state of practice.
- Board certification in a relevant specialty (e.g., family medicine, internal medicine, gerontology).
- A minimum of 1 year of experience as a Nurse Practitioner or Physician Assistant, preferably in a home health or primary care setting.
- Strong computer skills and familiarity with electronic medical records (EMRs).
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse patient populations.
