About the Opportunity

The Registered Nurse (RN) - Home Health is a key member of our interdisciplinary team, responsible for providing high-quality, patient-centered care to individuals in the comfort of their own homes.

As a skilled nursing professional, you will work collaboratively with our team of healthcare experts, physicians, and family members to develop and implement comprehensive care plans that meet the unique needs of each patient.

Key Responsibilities:

  • Provide safe and effective nursing care to patients in their homes
  • Assess patient conditions and implement care plans in collaboration with the interdisciplinary team
  • Deliver wound care, medication management, IV therapy, and other skilled nursing services as needed
  • Educate patients and caregivers on disease management, prevention of complications, and self-care techniques
  • Collaborate with physicians and other healthcare professionals to develop and implement care plans
  • Document patient care in a timely and accurate manner
  • Participate in quality assurance and performance improvement activities
  • Attend and participate in interdisciplinary team meetings as needed

Requirements:

  • Active Florida RN license
  • At least 1 year of nursing experience, with Home Health experience preferred
  • Ability to work independently with minimal supervision
  • Strong clinical and critical thinking skills
  • Excellent communication and interpersonal skills
  • Reliable transportation
  • Ability to work on an as-needed basis

Physical Demands:

The RN - Home Health must be able to perform physical activities that require considerable use of arms, legs, and moving the whole body, such as lifting, balancing, walking, stooping, and handling of materials.

The RN - Home Health must also be able to navigate different home environments and adapt to various indoor and outdoor conditions.
