Responsibilities- Assess and Develop Care Plans: Conduct thorough patient assessments, develop and implement individualized care plans, and coordinate with healthcare teams to ensure seamless care delivery.
- Coordinate Resources: Effectively manage assigned caseloads, coordinate resources, and ensure timely delivery of services.
- Maintain Accurate Documentation: Maintain accurate and up-to-date patient records, ensuring compliance with regulatory requirements and organizational standards.
- Foster Collaborative Relationships: Develop and maintain strong relationships with healthcare teams, patients, and families, promoting a culture of respect, empathy, and open communication.
- Active NYS RN License: Possess an active and unrestricted New York State Registered Nurse (RN) license.
- Infusion and Home Health Experience: Previous experience in infusion and home health care is preferred.
- IV Therapy and Infusion Skills: Possess extensive IV therapy and infusion skills, with a strong understanding of infusion protocols and procedures.
- Excellent Communication Skills: Demonstrate excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Valid Driver's License: Possess a valid driver's license and reliable transportation (not required for Manhattan-based positions).
- Microsoft Office Suite Proficiency: Possess proficiency in Microsoft Office Suite, including Word, Excel, and PowerPoint.
- Bilingualism: Bilingualism is a plus, with the ability to communicate effectively with patients and families from diverse linguistic and cultural backgrounds.
Centers Plan for Healthy Living offers a competitive salary of $130.00 per visit, as well as a comprehensive benefits package, including health insurance, retirement savings, and paid time off.
Job TypeThis is a fee-for-service position, requiring travel to various locations throughout the five boroughs.
