Trinity Health FirstChoice is seeking a skilled Travel Nurse Manager RN to join our team in a Case Management role.
Key Responsibilities:- Assess, plan, evaluate, and coordinate management of patient care, including discharge planning and utilization needs.
- Support ministry and system goals for capacity management through achieving length of stay goals, identification of avoidable delays, and other Case Management initiatives.
- Coordinate with the interprofessional team to meet patient care goals.
- Establish partnerships with site Patient Experience champions to align efforts with patient satisfaction outcomes.
- Identify discharge needs, determine the appropriate level of care, coordinate planning, and problem solve continuity issues to ensure seamless continuum of patient care and prevent readmission.
- Ensure all appropriate clinical information is provided to outside agencies.
- Communicate pertinent information to the patient care team effectively and professionally.
- Identify and eliminate barriers to effective patient stay management and promote improvements in work processes within the department, hospital, and community.
- Pursue professional development opportunities and maintain licensure.
- Advocate for patient legal, social, and ethical rights while balancing patient resources and hospital financial stewardship.
- Refer quality issues and poor utilization of services to the appropriate sources.
- Coordinate/facilitate patient/family education.
- Educate hospital leadership and colleagues on current case management practices, patient coordination issues, and unit-specific trends.
- Provide consultation to professional colleagues and employees regarding patient stay options, financial considerations, psychosocial concerns, risk management, and ethical issues.
- Document pertinent assessments, interventions, transitions, and outcomes for patients in accordance with documentation standards, policies, procedures, and/or guidelines.
- Utilize computer systems.
- Maintain a working knowledge of applicable Federal, State, and local laws and regulations, Trinity Health Integrity and Compliance Program, and Code of Conduct.
- Valid license in the state(s) you will work.
- Minimum of 2 years of experience, depending on specialty area.
- Cannot hold a concurrent position at a Trinity Health facility.
