We are seeking a skilled Physical Therapist to join our team at Alternate Solutions Health Network LLC. As a Per Diem Physical Therapist, you will work with patients in their homes, providing personalized physical therapy services and helping them achieve optimal recovery outcomes.
About the Role
This is a per diem/PRN opportunity that allows you to choose your own schedule and work as much or as little as you like. You will have the flexibility to select from a variety of shifts and locations, ensuring that you can balance your work and personal life effectively.
Responsibilities
- Provide high-quality physical therapy services to patients in their homes, following treatment plans developed by physicians and therapists.
- Evaluate patient progress, adjusting treatment plans as needed to ensure optimal outcomes.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Maintain accurate records of patient interactions, treatment plans, and progress.
Requirements
- Current license as a Physical Therapist in the state of employment.
- Minimum one year of experience as a PT in an acute setting.
- Home care experience preferred.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
