Salary Range: $17.50 - $27.25 per hour, based on experience and qualifications.

 

Job Summary:

 

We are seeking a Care Coordinator Specialist who will strive to improve the quality of life for patients/residents by providing appropriate, meaningful activities and interventions on a one-on-one and/or group basis. The ideal candidate will administer the monthly activity calendar, elicit patients/residents input into activity planning, and maintain activities well organized and upbeat.

 

Key Responsibilities:

  • Plan and implement activities that promote socialization, engagement, and overall well-being among patients/residents.
  • Administer the monthly activity calendar and ensure activities align with patient/resident needs and preferences.
  • Elicit feedback from patients/residents to inform activity planning and implementation.
  • Maintain accurate records of activity participation and progress.
  • Collaborate with interdisciplinary teams to develop and implement individualized plans of care.

 

Qualifications:

 

We are looking for a highly motivated and compassionate individual with a strong background in social services or creative activities. A high school diploma or equivalent is required, and experience working with geriatric populations is preferred. Basic Life Support (BLS) certification is also required.
