About the Role

We are seeking an experienced MDS Nurse to join our team at Ciena Healthcare. As a key member of our care management team, you will be responsible for conducting assessments and care plan coordination for residents assigned to you.

Responsibilities
  • Complete the MDS, CAA's, and care plans within regulated time frames.
  • Assess residents through physical assessment, interview, and chart review.
  • Discuss resident care needs with caregivers, including physician, nursing, social services, therapy, dietary, and activity staff.
  • Review information from hospital, consults, and outside agencies and use such information in the completion of the assessment and care planning.
  • Coordinate, identify, and/or initiate significant change MDS.
  • Prepare for and conduct PPS meetings, maintaining MDS assessments per Medicare schedule and monitoring Medicare days and RUGs utilization.
  • Remain current with American Association of Nursing Assessment Coordinators (AANAC) requirements.
Qualifications
  • Registered Nurse, RN or Licensed Practical Nurse.
  • AANC certification a plus. RAC-CT.
  • Knowledge of the Resident Assessment Instrument (RAI) process, including the principles of the Prospective Payment Process (PPS) strongly preferred.
  • Experience as an MDS Nurse.
