About the Role

We are seeking a dedicated and compassionate Licensed Practical Nurse to join our interdisciplinary team at UofL Health - Frazier Rehabilitation Hospital - Brownsboro.

Job Summary

This role offers an opportunity to make a meaningful difference in the lives of patients recovering from stroke, brain injury, neurological conditions, trauma, spinal cord injury, amputation, and orthopedic injury. As a Licensed Practical Nurse, you will provide high-quality patient care, working collaboratively with our rehabilitation programs to deliver comprehensive physical medicine and rehabilitation services.

Key Responsibilities
  • Demonstrate expertise in medication administration and management, ensuring safe and effective treatment for patients.
  • Conduct thorough patient assessments, identifying individual needs and developing personalized plans of care.
  • Collaborate with our multidisciplinary team to develop and implement treatment plans, prioritizing patient-centered care.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Participate in quality improvement initiatives, promoting excellence in patient outcomes and experience.
Qualifications and Requirements
  • Graduate of an accredited licensed practical/vocational nurse program.
  • Current state licensure as a Licensed Practical/Vocational Nurse.
  • CPR/BCLS certification; ACLS preferred.
  • IV Certified or obtained within 6 months highly preferred.
  • Minimum six months' Medical/Surgical experience in an acute care setting preferred.
