Job Description

The Home Health Risk Assessment Nurse Practitioner - PRN/Part Time Opportunity is a rewarding and challenging role that offers the chance to make a real difference in the lives of others. As a member of our team, you will have the opportunity to work independently, conducting comprehensive health assessments and developing personalized care plans for patients in their own homes.

This role requires strong clinical skills, excellent communication abilities, and a passion for delivering exceptional patient care. You will be working in a variety of settings, including private homes, skilled facilities, and telehealth platforms, providing flexible and convenient access to healthcare services for our patients.

In this role, you will have the autonomy to work as an independent contractor, selecting the cases that best fit your schedule and preferences. You will also have access to a range of resources and support, including training, technical assistance, and peer review, to help you succeed in your role.

Responsibilities
  • Conduct comprehensive health assessments, including medical history, physical examination, and laboratory tests, to identify risk factors and develop personalized care plans
  • Develop and implement individualized care plans, taking into account the patient's medical history, social determinants, and treatment goals
  • Provide education and support to patients and their families, empowering them to take an active role in managing their health and well-being
  • Collaborate with healthcare providers and other stakeholders to ensure seamless transitions of care and optimal outcomes
  • Maintain accurate and timely documentation of patient encounters, assessments, and interventions
Qualifications

To be successful in this role, you will need to possess the following qualifications:

  • Master's degree in nursing or a related field
  • Current RN licensure in the state of practice
  • NP licensure and certification in a relevant specialty
  • At least one year of experience as a nurse practitioner
  • Strong clinical skills, excellent communication abilities, and a passion for delivering exceptional patient care
