About the Role

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Better Home Care Agency. As an LPN, you will play a vital role in providing high-quality care to our patients in the comfort of their own homes.

Key Responsibilities
  • Provide direct, individualized nursing care to patients based on their unique needs and under the supervision of our Director of Nursing.
  • Ensure the safe and effective delivery of home health services to patients and their families.
  • Implement care plans formulated by physicians and registered nurses.
  • Document patient care and progress accurately and in a timely manner.
  • Assess and educate patients and their caregivers on self-care activities and disease management.
  • Maintain a safe and healthy work environment and adhere to company clinical standards.
  • Collaborate with our clinical team to coordinate home health services.
Requirements
  • Must be at least 18 years old and licensed to practice as an LPN in the State of PA.
  • One year of clinical experience or other relevant experience.
  • Certification in CPR for healthcare professionals.
  • Valid driver's license, proof of insurance, and vehicle registration.
  • Current PPD or Chest X-Ray.
  • Eligibility for State Police Criminal Record Check and Child Abuse History Clearance.
  • FBI clearance.
  • Ability to work independently and manage time effectively.
  • Ability to travel to patient locations as assigned.
