You will work collaboratively with management staff, physicians, administration, and other stakeholders to promote quality improvement initiatives and provide referrals/support for follow-up care for uninsured or underinsured patients.

About the Requirements

To be considered for this role, you must hold a current RN license from the Oklahoma Board of Nursing and have a diploma from an accredited school/college of nursing or equivalent professional licensure at the time of hire.

Prior experience as an RN Care Manager, Hospice Nurse, Home Health Nurse, or Patient Navigator is preferred, as is a willingness to travel within 40 miles of Tulsa for occasional home visits and specialty appointments.

BLS certification from the American Heart Association or American Red Cross is highly valued, although not essential for this role.

The estimated annual salary for this position is $65,000-$80,000, depending on qualifications, experience, and education level.
