Job Summary
Lyon-Martin Community Health Services seeks a dedicated Mental Health Specialist to join our team. The ideal candidate will possess excellent clinical skills, experience working with diverse populations, and a commitment to providing high-quality care.

Key Responsibilities:
  • Evaluate, diagnose, and treat mental health/substance use disorders in individual client sessions.
  • Provide short-term case management to assigned patient panel, assisting clients with accessing benefits or support services as needed.
  • Crisis intervention during clinic hours of operation, monitoring the crisis triage Team chat, responding quickly and intervening with in mental health emergencies or high emotional distress.
  • Rotate on-call coverage for After Hours crisis calls for clients who are in extreme mental distress.
  • Engage clients who have broken community agreements in accountability conversations using a restorative justice lens.
  • Link clients into SUD care, pain management, Traditional Chinese Medicine, or Physical Therapy, or higher levels of care as appropriate.

Performance and Quality Improvement:
Meet benchmarks for applicable visit numbers and quality indicators. Document visit notes and client interaction in an accurate, detailed, and timely manner. Respond to in-basket messages within 72 hours. Maintain professional competence and highest possible ethical standards. Work collaboratively as part of an integrated healthcare team. Participate in weekly team meetings and monthly All Staff meetings. Meet contract and grant deliverables. Assist in continuous quality improvement through ongoing problem/needs identification and problem solving. Demonstrate cultural humility and sensitivity when interacting with diverse populations. Collaborate positively with other team members, giving and accepting constructive criticism.
