Job Summary:
Assess, develop, plan, implement, and evaluate patients' discharge planning needs to next-level care in collaboration with an interdisciplinary team.
Key Responsibilities:
- Ensure compliance with Condition of Participation (CoP) discharge planning.
- Identify and escalate gaps/barriers related to inpatient and observation length of stay.
- Assess patients at high risk for readmission, identify root causes, and develop plans to prevent readmission.
- Collaborate with the multidisciplinary team to ensure seamless discharge planning.
- Participate in opportunities for improvement in standard work and contribute to a culture of excellence.
- May also be responsible for utilization management, ED case management, admission processes, and serving as a liaison with post-acute providers.
Requirements:
- Graduate of an accredited school of nursing or equivalent education for licensure.
- Three years' acute care experience.
