Position Overview: The Perinatal and Community Access Nurse will spearhead initiatives aimed at educating expectant mothers and families dealing with opioid use disorders through an established educational framework. This role will also involve implementing and replicating clinic activities via a mobile health outreach program on a weekly basis.

COMPENSATION: $30.40-$32.38/hour based on experience, plus a $5,000 retention bonus.

EMPLOYMENT TYPE: Full-Time

Essential Duties and Responsibilities:

  • Conduct patient triage, including vital signs, health history, and medication reconciliation.
  • Document patient data in the Electronic Health Record in compliance with established standards.
  • Provide educational resources to patients and families, focusing on self-care tailored to their needs.
  • Administer medications as prescribed by healthcare providers.
  • Prepare examination rooms with necessary equipment and supplies.
  • Assist healthcare providers during examinations and procedures as needed.
  • Perform laboratory testing and maintain accurate logs of all diagnostic tests.
  • Administer vaccines according to current guidelines.
  • Engage in team huddles with primary care staff.
  • Manage telephone communications and medication refill requests.
  • Utilize electronic medical records effectively for patient documentation.
  • Keep patient medical records current for audits and reviews.
  • Participate in quality assurance and improvement initiatives.
  • Collaborate with primary care teams to ensure comprehensive patient care.
  • Communicate professionally with patients and team members.
  • Follow up with patients regarding lab results.
  • Identify patients needing specialized treatment services.
  • Assess health literacy and provide appropriate educational materials.
  • Conduct medication reviews with patients upon request.
  • Develop and distribute patient education materials.
  • Assist with community outreach activities, including health fairs.
  • Contribute to the development of outreach programs targeting pregnant women and families with behavioral health challenges.
  • Collect and report data on prenatal education class participants.
  • Perform nursing duties for patients scheduled with the mobile unit.
  • Maintain the mobile unit's cleanliness and supply inventory.
  • Adhere to all organizational policies and procedures.
  • Engage in performance optimization and risk management activities.
  • Support One Health's Compliance Program.
  • Deliver exceptional customer service, embodying the mission of One Health.
  • Complete additional tasks as assigned.

Minimum Qualifications:

Education/Certificates/Licenses:

  • Nursing degree from an accredited institution.
  • Current nursing license in the state of practice.
  • BLS Certification upon hire.
  • Valid driver's license.

Experience:

  • Preferred two to five years of experience in a primary care clinical setting.
  • Experience with Medication-Assisted Treatment for opioid use.
  • Background in clinical obstetrics.
  • Experience in behavioral health disorders and treatment.
