About the Role

Ciena Healthcare is seeking an experienced MDS Coordinator to join our team. As a key member of our skilled nursing team, you will be responsible for overseeing the RAI process and conducting assessments and care plan coordination for our guests.

Key Responsibilities
  • Complete the MDS, CAA's, and care plans within regulated time frames
  • Coordinate scheduling the RAI process with the interdisciplinary team
  • Assess residents through physical assessment, interview, and chart review
  • Discuss resident care needs with caregivers, including physician, nursing, social services, therapy, dietary, and activity staff
  • Review information from hospital, consults, and outside agencies and use such information in the completion of the assessment and care planning
  • Coordinate, identify, and/or initiate significant change MDS
  • Conduct PPS meetings and maintain MDS assessments per Medicare schedule and monitor Medicare days and RUGs utilization
  • Remain current with American Association of Nursing Assessment Coordinators (AANAC) requirements
Requirements
  • Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensure
  • AANC certification a plus. RAC-CT
  • Knowledge of the Resident Assessment Instrument (RAI) process, including the principles of the Prospective Payment Process (PPS) strongly preferred
  • Experience as an MDS Nurse
