- Conduct comprehensive psychosocial assessments and develop individualized treatment plans.
- Lead individual, group, or family therapy sessions as needed, ensuring effective treatment outcomes.
- Collaborate with multidisciplinary teams to coordinate patient care and services.
- Maintain accurate and timely documentation of therapy services provided, adhering to facility and professional standards.
- Manage a caseload of patients, prioritizing their needs and ensuring timely interventions.
- Participate in treatment team meetings, contributing to the development of comprehensive treatment plans.
- May provide services in inpatient or outpatient settings, adapting to the needs of patients and the organization.
- Perform case management, discharge planning, and aftercare services as required.
- Demonstrate a patient-centered approach, prioritizing empathy, professionalism, and safety in all interactions.
- Master's degree in Social Work, Psychology, Counseling, or a related field.
- At least one year of experience working with comparable patient populations.
- Current licensure as a LMFT, LPC, LMSW, or LCSW.
