- Providing direct patient care and implementing personalized care plans
- Documenting patient information accurately and timely
- Assessing and educating patients and their families on self-care techniques
- Assisting RNs and physicians with specialized procedures and duties
- Participating in the coordination of home health services and reporting identified needs to the registered nurse and/or Clinical Supervisor
To be successful in this role, you will need to have:
- Eligibility to work in the United States
- CPR Certification and PPD Certification
- At least 1 year of experience as an LPN
- A valid LPN License with at least 1 year of work experience
- A valid Driver's license and reliable transportation
