Job DescriptionWe are seeking a Registered Nurse with 2+ years of clinical experience to join our team. As a Palliative Care Specialist, you will provide supportive care to patients with life-limiting illnesses and their families. Your key responsibilities include:
- Evaluating patient needs and creating personalized care plans
- Collaborating with interdisciplinary teams to ensure exceptional patient outcomes
- Communicating effectively with patients, families, and care teams
To be successful in this role, you must have:
- A current state license as a Registered Nurse
- 2+ years of nursing experience in a clinical setting (hospice experience preferred)
- Reliable transportation and ability to lift at least 80-100 lbs
