Key Responsibilities:
Assist clients with Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs) as directed by the Nurse Case Manager and tailored care plans.
ADLs may encompass, but are not limited to, personal hygiene, dressing and grooming assistance, eating support, mobility aid, and toileting help.
IADLs may include, but are not limited to, meal preparation, light housekeeping, running errands, accompanying clients to appointments, and aiding with communication tasks such as using the telephone.
Additional responsibilities may be assigned as needed.
