Job Summary

WellBe Senior Medical is seeking a compassionate and skilled Senior Geriatric Care Coordinator to join our team. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality care to our elderly patients. Your expertise in social work and geriatric care will enable you to develop and implement personalized care plans that address the unique needs of our patients.

Key Responsibilities
  • Conduct thorough assessments of patients' social and behavioral needs, identifying areas for improvement and developing targeted interventions.
  • Collaborate with our multidisciplinary team to coordinate care and ensure seamless transitions between healthcare settings.
  • Develop and maintain relationships with community resources and service providers to ensure access to necessary support services.
  • Provide education and support to patients and their families, empowering them to make informed decisions about their care.
  • Document all interactions and interventions in our electronic health record system, ensuring accurate and timely documentation.
  • Participate in regular team meetings and contribute to the development of our social work program.
Requirements
  • Master's degree in Social Work (MSW) from an accredited program.
  • At least two years of experience in social work, preferably in a healthcare setting, with a focus on geriatric care.
  • Strong knowledge of community resources and services for the elderly and their families.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Ability to work independently and as part of a team, with a strong commitment to delivering high-quality care.
