Key Responsibilities- Assessment and Planning: Conduct thorough assessments of patients to identify their nursing care needs, develop individualized care plans, and prioritize tasks based on patient needs and available resources.
- Implementation and Evaluation: Implement nursing interventions safely and competently, provide ongoing evaluation of patient responses to care, and modify care plans as needed in collaboration with the healthcare team.
- Communication and Collaboration: Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination and patient satisfaction.
- Education and Training: Identify patient learning needs and provide education and training to promote health and wellness, as well as to support patient self-care and independence.
- Quality and Safety: Adhere to Kaiser Permanente's safety standards and guidelines, including infection control and patient safety protocols, to ensure a safe environment for patients and staff.
- Education: Associate's degree in Nursing from an accredited program.
- License and Certification: Registered Nurse License (Washington) required at hire, Basic Life Support required at hire.
- Skills and Experience: Ambulatory care nursing experience, primary nursing or case management experience, and strong communication, problem-solving, leadership, and decision-making skills.
- Ambulatory Care Experience: Previous experience in ambulatory care settings, preferably in gastroenterology or endoscopy.
- Primary Nursing or Case Management Experience: Experience in primary nursing or case management, with a focus on patient-centered care and coordination.
- BSN Degree: Bachelor of Science in Nursing degree, with a focus on leadership and management.
