Job Summary

Key Responsibilities
  • Administer medications and document patient care in accordance with established protocols and guidelines.

  • Provide education and support to residents and their families on health-related topics and disease management.
  • Participate in quality improvement initiatives to enhance the overall quality of care.
  • Document patient information accurately and thoroughly in our electronic medical record system.
Requirements
  • Diploma in Nursing from an accredited school of nursing.
  • Current licensure as a Licensed Practical Nurse in the State of Wisconsin.
  • CPR certification.
  • Minimum one year of experience in a long-term care setting.
